چکیده

Background Patients with hypertrophic cardiomyopathy (HCM) develop left ventricular outflow tract (LVOT) obstruction due to combination of asymmetric septal hypertrophy and systolic anterior motion of the mitral leaflet. Degree of LVOT obstruction is an important predictor in patient symptoms and clinical outcome. LVOT obstruction is currently assessed by Doppler echocardiography (echo) using peak velocity to calculate pressure gradient in the LVOT. 4D phase contrast (4D PC) MRI is an emerging technique for quantification of regional flow and velocity. 4D PC MRI is a 3 dimensional, 3 directionally encoded, time resolved (cine) velocity acquisition. It has the advantage of providing full 3D volume coverage and flexibility of retrospective analysis of flow at any location and in any imaging plane. In this preliminary study, we

Ascending aorta flow derangement is a marker of outflow obstruction in hypertrophic cardiomyopathy

Background Alterations in systolic blood flow are a characteristic finding in patients with obstructive hypertrophic cardiomyopathy (HCM). Echocardiography is the gold standard for hemodynamic assessment in HCM, but this technique is generally limited to measurement of the left ventricular outflow tract (LVOT) pressure gradient and may not provide a complete description of the hemodynamic impac...

Significance of left ventricular outflow tract cross-sectional area in hypertrophic cardiomyopathy: a two-dimensional echocardiographic assessment.

The morphologic determinants of subaortic obstruction in patients with hypertrophic cardiomyopathy are not completely understood. To define the relation between left ventricular outflow tract orifice size and presence or absence of subaortic obstruction, we studied 65 patients with hypertrophic cardiomyopathy and 16 normal controls by quantitative two-dimensional echocardiography. Left ventricu...

Influence of anterior mitral valve length and septal wall thickness on the prevalence of left ventricular outflow tract obstruction in hypertrophic cardiomyopathy

Background Interest has emerged in the evaluation of mitral valve morphology in patients with Hypertrophic Cardiomyopathy (HCM) and its contribution to left ventricular outflow tract obstruction (LVOTO). Specifically, elongation of the anterior mitral valve leaflet (AMVL) appears to be part of phenotypic expression in HCM and has been associated with severity of LVOT obstruction. However, the i...

The diagnosis of left ventricular outflow tract obstruction in hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy is a prevalent genetic disease characterized by left ventricular hypertrophy, presenting dynamic obstruction of outflow tract with subaortic gradient happening at rest in 30% of the cases. It is attributed to the intricate interaction between the anterior mitral leaflet, the interventricular septum and altered flow vectors generated in left ventricle along with chang...

Flail Subaortic Membrane Mimicking Left Ventricular Outflow Tract Obstruction in Hypertrophic Cardiomyopathy

A subaortic membrane is an uncommon cause for left ventricular outflow tract obstruction. Hypertrophic cardiomyopathy with dynamic left ventricular outflow tract obstruction would mask the presence of the subaortic membrane on transthoracic echocardiography and cause a false diagnosis. We report a patient with subaortic stenosis due to flail subaortic membrane misdiagnosed as obstructive hypert...
